My Surgical Approach

Dystonia is a complex condition that requires patience from both the surgeon and the patient.

Unlike Parkinson's or Essential Tremor, where results are often seen instantly, the brain's response to DBS in Dystonia takes time. It is a gradual process of rewiring the motor circuits.

The gold-standard target for Dystonia is the Globus Pallidus Interna (GPi). By stimulating the GPi, I reduce the excessive, involuntary muscle contractions and twisting postures that define the condition.

When I Recommend DBS

Primary Generalized Dystonia (especially DYT1 positive)
Severe Cervical Dystonia (Torticollis) failed Botox
Tardive Dystonia resulting from medication side effects
Secondary Dystonia with significant structural brain damage

Types of Dystonia Treated

  • Generalized: Affecting the whole body. Most responsive in pediatric patients.
  • Cervical: Affecting the neck and shoulders. Dramatic relief for Torticollis.
  • Tardive: Often responds very well to GPi DBS.

The "Gradual" Effect

I advise all my Dystonia patients to expect a 3 to 6-month journey. The muscle memory of years of twisting does not vanish overnight. We programme the device in small increments, allowing the brain to adapt.

Questions

Dystonia DBS FAQ

Can DBS help my child with Dystonia?
Yes. Pediatric patients with primary generalized dystonia are often the best candidates for DBS. Early intervention can prevent permanent skeletal deformities.
What if I have Cervical Dystonia?
If Botox injections are no longer working or causing side effects, GPi DBS is a highly effective second-line treatment.
Will I see results immediately?
Typically no. While some relaxation is seen early, the full clinical benefit usually takes several months of consistent stimulation and programming.
What is the success rate for Dystonia?
Success depends on the type of Dystonia. Primary generalized cases often see 50-80% improvement in posture and movement.
